Member of the Month Nominations

This form will be submitted by a member on their own behalf or for another member in the community. This form will be used to nominate a fellow mHUBian that expemplifies the values of community, collaboration, innovation and participation within the mHUB community. Nomination forms will be scored on a rubric created to correspond with the survey questions to determine the winner which will be announced on the first monday of each month. Winner of member of the month will have the closest parking spot to the front door which will be labelled by a plaque, and lunch provided by mHUB on the second and fourth Friday of the month along with other mHUB Swag!!
